This survey can be used as a pre-/post- evaluation, or a simple way to get to know where your students.
For my own classes, I uploaded the questions to a survey generator online, then provided students the link. I did this at the beginning and end of the school year.

Weekly Homework Template
I use this Two-A-Day Weekly Homework template with my students who ESL students. Students have two questions a day that are either a review from the previous day/weeks material.
