As you sort the colors from your package you ask a friend a question from the colored circle that corresponds with that color of M and M. Guitar Cut-out (Interest Inventory)
Guitar Cut-out is sized to fit standard sized paper. It is formatted on Microsoft Excel (you need this program to open the file!). It is in 2 different slides/pages, so the top has to be pasted to the guitar body. This is an editable download in Microsoft Publisher. I used the font Better Together for the headers.
I hand this out on the first day of school, and give the kiddos some class time to fill it out. I allow them to sit by whoever they want while they work on it, and so it works in two ways - I get to learn more about them, and also get to see who they sit by so I can make a seating chart that keeps chatty friends from each other since I have kids I don't know.
