Description

Created for Tier 3 RTI students who require intensive behavioral intervention or for students with a Behavior Management Plan in their IEPs.

Some students require frequent feedback and reward. For these kids, an individual behavior plan is needed.

All 3 colors and a black and white version are included in the editable template! If you have specific behaviors you want to address, this is for you!

I recommend that you install the following fonts on your computer if you want the text to look the same. Both fonts are free for personal use:
1) KG Second Chances Solid
2) CKbig


HOW TO USE THIS PLAN
Print the behavior dial of choice. Cut out the circle and arrow. Fold the circle in half. Laminate the behavior dial and arrow. Punch a hole in both. Use a brad (brass paper fastener) to connect the arrow to the dial.

The student begins each day at 0. For every positive behavior, the student moves up the dial. For every negative behavior, the student moves back on the dial. Have a list of concrete examples of both positive and negative behaviors and make sure both student and staff know them.

Whenever the student reaches +3, the student gets a sticker (sticker book included on page 7).

If the student reaches -3, there is a consequence (loss of privileges, phone call home, etc.)

In the beginning, try to reward the student frequently. As time passes, you can prolong the time in between rewards.

I created this for my own students with Behavior Management Plans in their IEPs and they love it!
