Health Check Form

Use this form when completing a health check assessment on a child at a licensed childcare facility, preschool, in home daycare, or anywhere else where childcare is provided. This form is a great resource for both the provider and the parent and informs the parent of health issues that were examined by the provider or staff and how/where/when the child was examined.
