This editable Google Doc shows an annotated bibliography in MLA format, including the header and page number on the top right, name, teacher name, class, and date on the top left, title, and then the MLA citation and 3 paragraphs required in an annotated bibliography. The paragraphs are also color coded. This template also includes a place for students to include a thesis statement, which you can include or omit.

You can give this as an example, a template, or both. This has been extremely helpful to show my students what their annotated bibliography is supposed to look like.
