Parent Survey Editable in English & Spanish + Google Forms

This parent survey comes in a bilingual editable version in MS Word and a Google Form option too!

My school has a dual language program and we have many bilingual families who speak primarily Spanish at home. This resource has been very helpful in establishing initial communication with all of our parents.

This survey is also a great way to get a quick snapshot of your new students by having their parents fill out this survey at the beginning of a new school year, or whenever you receive a new student.

I give this parent survey out at Parent Orientation before school starts, but you could also give it out on the first day of school, at Back to School night, email it out or post it on your classroom website.

This survey is designed to fit on one piece of paper so you can print it with English on one side and Spanish on the other side.

These are the questions and requests for information that are included:

• Name
• Birthday
• Parent/guardian custody
• Siblings at school
• With whom and where a student goes after school
• Where the student needs help according to the parent/guardian
• Parent expectations for their child and teacher
• Holidays not celebrated
• Allergies
• Parent contact info
• Permission to contact by text
• Technology available at home (Updated August 2020)
• On Google Forms only: Anything else you would like me to know?

Finally, at the end of the page, there is a place to put your teacher name and room number.

*A note about the Spanish: I used words and phrases that are common and familiar to my students' parents. There may be regional differences in word choice and usage depending on what countries your students and their families are from.
