Description
This is an INFORMAL questionnaire that can be given to teachers and parents when social skills are an area of concern. I use this form prior to starting students in small group social skills lessons, as a pre-test and to help guide my lessons. I use it again after 6 weeks to determine student growth and continued areas of concern.
