How to use these Hearing Screening Forms:
- Edit. These hearing screening forms are editable to add student information, dates, and notes. You can also edit parent letters with student information and add your signature.
- Print. Print preferred hearing screening forms and accompanying letters (choose between 20 dB and 25 dB testing levels) on white printer paper.
- Cut. Half-page screenings and letters have a dotted line to make for easier cutting.
- Use. Simply fill in each screening form during or after testing. If you choose not to edit the forms, it is easy to print a handful of screening forms and fill them in as you complete screenings.
