This creative and engaging package includes the following:

⭐Who Am I? Activity: Students fill out this form to provide you with important information about themselves. After students fill out these forms, I place them in a binder in my room so I can refer back on them throughout the year.

⭐Parent Perspective Information Form: I have students take this form home to their parents to fill out and return. It includes important information about them (email address!!!), and their hopes for their child. Like the “Who I Am?” Form listed above, I place these forms in a binder in my room so I can refer back to them throughout the year.

⭐Reading and Writing Surveys: Four pages of Reading and Writing surveys are included, to assess student interest and attitudes towards reading and writing. This is a great way to learn about who your students are as readers and writers. I have my students glue these pages into their Interactive Notebooks.

⭐Reading Interest Survey: Students fill out this form to show the types of books that most interest them. This is really helpful to refer to when assigning new novels or Reading Groups! I have my students glue this sheet into their Interactive Notebooks.

⭐Back to School Interest Survey: Two pages of questions for students to answer to help you get to know your students right from the very first day.

⭐Reading Group Musical Chairs: A fun and effective way to have students sample and record information on a large group of novels in a short period of time. This is a really great way to assign reading groups. Detailed information on how to do the activity with your class, as well as a form for your students to fill out while completing the activity, are included.

⭐Two Truths and a Lie: Templates are included to complete this fun get-to-know-you ice breaker activity with your class.

⭐Would You Rather? Activity: A two page Would You Rather? Activity is included. Students choose their answers to fun and informative questions, then discuss their answers with the class.

⭐10 Interesting Facts About Me Activity: Students use the included template to list 10 interesting facts about themselves. This is a fun way to get to know your students, and makes for great classroom discussion.

⭐Digital EASEL Activities: A digital EASEL version of the "10 Interesting Facts About Me Activity", "Back to School Interest Survey", and "Who Am I?" Activity have been included. These already have text boxes added, so it is easy for students to simply type in their answers.
